Case Management

Your case manager will assist with coordinating your care throughout your inpatient stay as well as help you plan for your discharge. Case managers also work with your insurance company to obtain the proper authorizations and confirm coverage for services. Together with your interdisciplinary care team, the case manager helps establish your length of stay, discharge goals and recommendations for follow-up care. They will also assist you in obtaining any needed supplies or services to facilitate discharge. These could include:

  • durable medical equipment, such as a wheelchair or walker.
  • equipment to assist you with activities of daily living, such as commodes or shower chairs.
  • medical supplies, including wound-care supplies or urinary catheters.
  • follow-up appointments with primary and specialty medical providers.
  • coordination of appointments for specialized treatments, such as dialysis or chemotherapy.
  • ongoing outpatient rehabilitation.
  • home health care.
  • community and financial resources.
