As a new patient, you may be wondering how to begin. Here are some forms to help you get started. You may fax these directly to the Marianjoy Admissions Department at 630.909.8571. If you have any questions regarding the Marianjoy admission process, please call the Marianjoy Admissions Department at 630.909.8920.

Self-Referral

If you don't have a referral already from a physician, you can be evaluated by a Marianjoy physiatrist for admission to our facility. Please fill out this form to start the process.

Admissions Checklist

To help you plan for a smooth transition to Marianjoy Rehabilitation Hospital, we ask that you begin obtaining this information prior to admission.

Application for Community Care Assistance

Any patient with out-of-pocket expenses, whether insured or uninsured, can qualify for Marianjoy's charity care program. Uninsured patients could receive both the self-pay discount, and if they qualify, an additional discount from the charity care program. Marianjoy works with patients to establish payment plans as needed. You may be eligible to receive free or discounted care from Marianjoy Rehabilitation Hospital. Please fill out this form and attach indicated paperwork to help us determine your eligibility. Please see Billing and Insurance for more information.
