NM MyChart

As part of our continuing integration into Northwestern Medicine, we are pleased to share that Marianjoy Rehabilitation Hospital, part of Northwestern Medicine, and Marianjoy Medical Group will transition to Northwestern Medicine's electronic health record system on September 7. With this change comes a new patient portal, NM MyChart, which will make it easier for you to access your healthcare information and communicate with your care team.

Starting September 7, you will be able to open an NM MyChart account. After September 7, once you have had an appointment with one of our physicians, you will be able to request prescription refills, message your care team, pay your bill and view visit information in NM MyChart. (Until your next appointment, please continue to call your physician's office to communicate with your care team.)

Although your patient history will not be transferred into your new NM MyChart account, you will be able to view and print your prior medical history from your existing Marianjoy patient portal.

Signing up for NM MyChart is easy. Just follow these steps:

  1. Visit nm.org/mychart
  2. Follow the sign-up instructions to activate your account
  3. Create a username and password, and log in

For access to your healthcare information anywhere, anytime, from your mobile device, you can also set up and use NM MyChart through the MyNM app. To get the app, text MYNM to 43506 or find the MyNM app in the App Store or on Google Play.
