Case Management

Your Case Manager is a key member of your rehabilitation team and will be assigned to you at the beginning of your stay. This person is the liaison between you, your family, your physician, your rehabilitation team and your insurance company. He or she will attend the weekly care team meetings and discuss your progress and goals with your physicians and therapists.

An important role of the Case Manager is to work with you and your treatment team to develop a safe discharge plan to your next level of care. Your Case Manager will communicate your progress to you and your family and help keep you on target for your discharge goals. He or she will help you and your family understand the levels of rehabilitation care that are available for your condition once you leave Marianjoy and will help coordinate whatever services and equipment you may need.

Additionally, your Case Manager is responsible for the coordination with Medicare or your insurance company on your behalf, according to the requirements of your insurance plan. He or she will assist in accessing community or government resources as well.

Case Managers are either nurses or social workers with an extensive background in many clinical areas, including orthopedic, pediatric, brain injury, stroke, spinal cord, and neuromuscular conditions. They are well-versed in areas such as managed care, community resources, state and federal resource programs and programs for people with disabilities.
